    For Business Subsidies

    Here are the business subsidy report forms that must be used to report or update each calendar year's activity. The reports should be completed and submitted by April 1 of the following year.

    NOTE: Our fax machine is not available to submit Minnesota Business and Financial Assistance Forms. Please submit forms by e-mail to ed.hodder@state.mn.us or by mail Minnesota Department of Employment and Economic Development, Economic Analysis, 180 E 5th St Suite 1200, St. Paul, MN 55101.
