Qualification Checklists
Certification Applications
Power of Attorney
Consultants acting on behalf of a client who wish to file a certification application or annual report need to create an account. As part of the process, we'll send you this form.
Credit Allocation Application
Businesses and Investors/Funds must apply for a tax credit allocation before the investment transaction.
Proof of Investment
Businesses need to provide a proof of the investment within 15 days of the investment transaction.
Annual Report
Program participants need to report annually (investors/funds for three years, businesses for five years).
Insolvency Report
File this report if your business ceases operation or becomes insolvent.
